📧 “SUPPLEMENTAL… REQUEST” to OSU
Ms. Bullis and Mr. Belton,
This letter supplements my public records request and preservation demand submitted April 2, 2026. I am writing to expand the scope of that request based on information I have since obtained regarding statements made by OSU DPS officers to medical staff at Good Samaritan Regional Medical Center during my detention.
NEWLY IDENTIFIED FALSE STATEMENTS IN MEDICAL RECORD
I have obtained my medical record from Good Samaritan Regional Medical Center dated April 1, 2026, which documents the following statements attributed to OSU DPS officers present at the time of my medical clearance evaluation:
1. That I was transported to the hospital having been "trespassed from OSU campus for verbal threats to an OSU Director." This is false. The Exclusion Notice issued in my case (Case 2025-00494) cites "Proscribed Conduct" defined as "Obstruction or disruption" — not verbal threats. No finding of verbal threats appears in any OSU documentation.
2. That "when he was asked to leave by the police he refused and was arrested." This is false. Audio recorded at the scene on April 1, 2026, documents that Officer Beeton did not ask me to leave campus prior to placing me under arrest.
These statements, if made by OSU DPS officers, constitute false reports to medical personnel that entered my permanent medical record and affected my care.
ADDITIONAL RECORDS REQUESTED
In addition to the records identified in my April 2 request, I hereby request:
1. All communications between OSU DPS officers and Good Samaritan Regional Medical Center staff on April 1, 2026, including any verbal statements documented in reports or notes, written communications, or radio communications related to my transport and medical clearance.
2. All records reflecting what information Officer Beeton or any other OSU DPS officer provided to Good Samaritan staff regarding the basis for my detention and my behavioral history.
3. All records reflecting any OSU DPS officer's decision to deny or restrict my access to a chaplain during my detention, including any communications with Good Samaritan security staff regarding chaplain access. I explicitly and repeatedly requested access to a chaplain during my detention; I am a life-professed member of a licensed religious order and my wife is an ordained minister. I was informed after the fact by a Good Samaritan security officer that the decision to deny chaplain access was made by OSU law enforcement personnel.
4. PHONE SEIZURE AND SEARCH: All records reflecting the identity of the officer who took physical possession of my personal cell phone during the arrest on April 1, 2026, and any records of what was done with the device while in officer custody. Audio recording of the incident ends abruptly at the moment an unidentified officer made contact with my phone. Officer Beeton believed recording was continuous throughout the detention; I only learned upon later review that it had stopped. If an officer navigated my phone's interface to stop the recording, that constitutes a warrantless search of a personal electronic device in violation of the Fourth Amendment. I request identification of the officer responsible and all records related to the handling of my phone while in custody.
PRESERVATION DEMAND
All records identified above are subject to the preservation demand issued in my April 2, 2026 request. This supplemental demand extends that preservation obligation to all records identified herein.
Please confirm in writing within five (5) business days that these additional materials have been preserved.
